The quick answer

For an ordinary UK passport application after a deed poll, prepare two kinds of evidence: the document linking your old and new names, and a separate record showing you use the new name. HM Passport Office normally accepts one suitable name-in-use document for a UK application, with exceptions for particular changes.

Your deed poll and your supporting record do different jobs

DocumentWhat it demonstratesExample
Name-change evidenceThe link from the old name to the new name.Your signed and witnessed deed poll.
Name-in-use evidenceThe new name is being used in everyday official records.A recent employer letter or eligible bank statement.
Identity or nationality evidenceYour entitlement and identity for the passport application.Documents requested by the application checklist.

Keeping these jobs separate makes the process easier to plan. The deed poll establishes the name link; updating an existing record then gives you evidence of use. Submitting another copy of the same deed does not provide an independent record of everyday use. HM Passport Office: evidence to change a name

Which name-in-use documents can you use?

HM Passport Office’s evidence policy lists the following examples. The first group must have been issued within the last 12 months. HM Passport Office: evidence to change a name

EvidencePractical check
Employer letter or employment recordAsk for the new name to be written clearly and the document dated.
Tax record or government letterUse an official document issued in the changed name.
Bank statementListed for UK applications; check overseas requirements separately.
Educational recordUseful for a student or child, such as a school report.
Utility bill or child benefit recordCheck it actually identifies the person whose name changed.
Valid driving licence or national identity cardMay be older than 12 months, but must meet HMPO’s issue-date and validity rules.

Evidence should show the part of the name that changed. An unchanged middle name may be omitted under HMPO’s policy. If you changed a middle name and the record shows only an initial, check the policy or ask HMPO about that evidence rather than guessing.

How to build your evidence in a sensible order

Settle the exact spelling and order of every name before ordering your deed poll.

Sign and witness the deed according to the instructions supplied.

Ask an existing organisation, such as your employer, bank or education provider, to update its records using the deed and its own identity checks.

Request an eligible dated document in the new name.

Check the spelling against the deed.

Prepare your passport application with both the linking evidence and the name-in-use record, plus everything on the application checklist.

What if your bank wants a new passport first?

Ask whether the bank can use your existing identity evidence together with the deed poll. Request its written requirements, particularly if the first answer simply says “new photo ID”. At the same time, consider a different existing record, such as an employer letter, that fits HMPO’s evidence list.

You do not have to prove that every organisation has already updated your name. The normal UK rule asks for one suitable record, subject to exceptions and further checks. If none is available, explain the difficulty to HMPO before submitting unsuitable evidence. A social-media profile or a self-written statement is not interchangeable with the official records listed in its guidance.

Start with the document linking your names

Use your signed deed poll to begin updating records, then gather a suitable document showing your new name in use for the passport application.

When the standard one-document rule changes

  • Minor changes to an adult name can follow a separate route requiring two pieces of name-in-use evidence.
  • Some changes supported by marriage, adoption or other specified documents have different requirements.
  • Applications from overseas follow country-specific document tables and can require more evidence.
  • Children need the relevant consent evidence as well as the name-change documents.

Use the route matching your actual change. See our child passport name-change guide for the additional family paperwork. HM Passport Office: small changes to forenames; HM Passport Office: evidence to change a name

Frequently asked questions

For an ordinary deed poll application, also prepare eligible evidence that you use the new name and the other documents on your application checklist.

No. HMPO normally accepts one suitable name-in-use document for a UK application, although some cases need different or additional evidence.

HMPO lists a bank statement issued within the last 12 months for UK applications. Overseas applications have separate requirements.

An eligible educational record can do so. It must show the changed name and meet the date requirements; parental consent is a separate issue.
